Application of Cognitive Behavior Therapy for Sexually Abused Children : Case Illustrations

In spite of the growing recognition of child sexual abuse, a standard treatment for sexually abused children has not yet been established in Korea. In this report, we introduce a modified individual cognitive behavioral therapy (CBT) program and illustrated two clinical cases applying our program. A standard 12-session CBT program was applied to two female children suffering post-traumatic stress disorder as a result of sexual abuse. The clinical changes were assessed by several psychological measures. Case A, offended by her biological father for 2 years, was focused to diminish guilty feelings about the disclosure and the resulting divorce of her parents. Case B, molested by the unacquainted man who broke into her house, was centered to correct distorted perceptions of others¡¯ reactions. Psychological measures revealed clinical improvement in depression and anxiety as well as in internalizing and externalizing scores in both cases. This case report showed the successful engagement of our CBT program for sexually abuse children, although both cases, in particular, had difficulties in generating alternative positive thoughts and practicing gradual exposure. A comparison study with large sample size are needed to confirm the efficacy of this program.
